Weekly Payroll Attestation

I attest and certify that have taken all breaks required by California Labor law during this pay period and all previous pay periods, detailed below.

I attest and certify that have been provided paystubs for each pay period that detail my wages prior to this pay period.

I attest and certify that I have not been injured on the job and that any injuries will be submitted to management in writing.

I attest and certify that I have reviewed my wages, hours, and compensation for all previous pay periods, and that I hereby confirm their accuracy.

I attest that I have no knowledge of any compensation due to me for wages other than as due for this pay period.

I hereby attest that I have received a copy of the company handbook, and that I am aware that a copy is available to me at any time in the office or on the company website.

Breaks Details

I understand that I am entitled to be relieved of all duty to take an unpaid work-free 30-minute meal period starting no later than the end of my 5th hour of work, and a second unpaid 30-minute meal period whenever I work more than ten hours, but that I can waive the second meal if I don’t work more than 12 hours.  I also understand that I am entitled to be relieved of all duty to take a paid 10-minute rest period for each work shift of 3.5 to 6 hours, two paid 10-minute rest periods for each work shift of 6 to 10 hours, and three paid 10-minute rest periods for each work shift of 10 to 14 hours.  I understand that I may not delay, skip or shorten any meal or rest breaks.  I declare under penalty of perjury that this time record accurately reports all the time I have worked during the covered pay period.  I am making this declaration freely and voluntarily, and I understand that the company will not retaliate against me if I have any questions regarding my meal periods, rest breaks or wages. I also certify that I have not been hurt on the job today.  

 

Entiendo que tengo derecho a estar libre de todas mis responsabilidades para tomar un periodo de comida sin pago de 30 minutos libres de trabajo empezando a más tardar al final de mi quinta hora de trabajo, y un segundo período de comida de 30 minutos sin pago cuando trabajo más de diez horas. También entiendo que tengo derecho a estar libre de todos mis deberes para tomar un período de descanso de 10 minutos con pago por cada turno de trabajo de 3.5 a 6 horas, dos períodos de descanso con pago de 10 minutos por cada turno de trabajo de 6 a 10 horas, y tres períodos de descanso de 10 minutos con pago por cada turno de trabajo de 10 a 14 horas. Entiendo que no retrasar, puedo omitir o reducir cualquier periodo de comida o descanso a menos. Declaro bajo pena de perjurio que este registro de tiempo informa con precisión todo el tiempo que he trabajado durante el período de pago cubierto. Estoy haciendo esta declaración libre y voluntariamente, y entiendo que la empresa no tomará represalias contra mí si tengo alguna pregunta acerca de mis períodos de comida, descansos o salarios.También certifico que hoy no me han lastimado en el trabajo.
